Sushruta Samhita Uttaratantra Chapter 23 Nasaroga Pratiṣedha (Treatment of Diseases of the Nose)


The 23rd chapter of Uttaratantra of Sushruta Samhita is named as Nasaroga Pratiṣedha Adhyaya. This chapter deals with the Treatment of Diseases of the Nose.

अथातो नासागतरोगप्रतिषेधं व्याख्यास्यामः ।।१।।
यथोवाच भगवान् धन्वन्तरि ||||

We will now expound the chapter by name Näsă roga pratiședha -treatment of diseases of the nose; as revealed by the venerable Dhanvantari.

Putinasya Cikitsā

Treatment Principles

पूर्वोद्दिष्टे पूतिनस्ये जन्तोः स्नेहस्वेदौ छर्दनं स्वंसनं ।।
युक्तं भक्तं तीक्ष्णमल्पं लघु स्यादुष्णं तोयं धूमपानं काले ||||

Useful therapies – The disease putinasya has been described earlier. In this disease, the below mentioned treatments are highly beneficial –

        Snehana – oleation

        Swedana – sudation

        Chardanam – therapeutic ememsis

        Sramsana – therapeutic purgation

Diet – The food given to the patient should be Tikshna – having penetrating propertly, alpa – less in quantity and laghu – light and easily digestible.

Other beneficial things – Those suffering from Putinasya should drink warm or hot water and inhale medicated smoke at suitable times, as and when needed.

Avapida, Nasya

हिङ्गु व्योषं वत्सकाख्यं शिवाटी लाक्षा बीजं सौरभं कट्फलं ।।
उम्रा कुष्ठं तीक्ष्णगन्धा विडङ्गं श्रेष्ठं नित्यं चावपीडे करञ्जम् ।।४।।
एतैद्रव्यैः सार्षपं मूत्रयुक्तं तैलं धीमान्नस्यहेतोः पचेत ।।

Below mentioned herbs are the best for administering Avapida Nasya – nasal drops, daily –

        Hiñgu,

        Vyosa

        Vatsaka

        Sveta (Punarnava)

        Laksa

        Seeds of Surabhi

        Katphala

        Ugra

        Kustha

        Tiksnagandha

        Vidanga

Method – The intelligent physician should cook and process Sarsapa Taila – mustard oil with the above-mentioned herbs along with cow’s urine. This oil, when prepared shall be stored and used for nasya – as nasal drops.

Nāsāpāka cikitsā

नासापाके पित्तहत्संविधानं कार्य सर्व बाह्यमाभ्यन्तरं ५॥
हृत्वा रक्तं क्षीरवृक्षत्वचश्च साज्याः सेका योजनीयाश्च लेपाः ।।

Principles of Treatmemnt

        Pittahara Vidhana – Nasapaka should be treated comprehensively by administration of external and internal therapies which would mitigate pitta.

        Raktamokshana – bloodlettingn

        Seka – pouring medicated liquids in streams over the face

        Lepa – Barks of Ksiri Vrksas – trees yielding milky sap and ghee are mixed together to form a paste and this is used for external application.

Treatment of Nasagata Raktapitta and Puyarakta

नासापाके पित्तहृत्संविधानं कार्यं सर्वं बाह्ममाभ्यन्तरं च ||५||
हृत्वा रक्तं क्षीरवृक्षत्वचश्च साज्याः सेका योजनीयाश्च लेपाः ||
वक्ष्याम्यूर्ध्वं रक्तपित्तोपशान्तिं नाडीवत्स्यात् पूयरक्ते चिकित्सा ||६||

Acharya says that he would explain the different treatment methods and measures to cure Nasagata Raktapitta – bleeding disorders expressing through the nose – nasal bleeding in the forthcoming chapter (45th chapter of the same section, Uttara Tantra).

He further states that Nasagata Puyarakta – a condition in which blood contaminated with pus (infected blood) is discharged from the nose) shall be treated on the lines of treating Nadivrana – sinuses.

Apart from this, the below mentioned therapies / remedies too shall be administered for the benefit of the patient –

        Vamana – therapeutic emesis, followed by

        Avapidana Nasya, along with

        Tiksna Dhuma – strong medicated smoking,

        Sodhana – cleansing therapies, and

        Nasya – nasal medication / nasal drops / errhines

Treatment of Ksavathu and Bramsathu

वान्ते सम्यक् चावपीडं वदन्ति तीक्ष्णं घूमं शोधनं चात्र नस्यम् |
क्षेप्यं नस्यं मूर्धवैरेचनीयैर्नाड्या चूर्णं क्षवथौ भ्रंशथौ च ||७||
कुर्यात् स्वेदान् मूर्ध्नि वातामयघ्नान् स्निग्धान् धूमान् यद्यदन्यद्वितं च |

The treatment principles of Ksavathu and Bramsathu include –

        Sirovirecana (head purging) – with the powder of head purging herbs filled in nadis (tubes)

        Swedana – sudation

        Sirovasti – oil pooling done on the head

        Snigdha dhumapana – unctuous smoking with vata mitigating herbs

Any other measures beneficial for the cure of the disease too shall be adopted.

Treatment of Dipta Roga

क्षेप्यं नस्यं मूर्धवैरेचनीयैर्नाड्या चूर्णं क्षवथौ भ्रंशथौ च ||७||
कुर्यात् स्वेदान् मूर्ध्नि वातामयघ्नान् स्निग्धान् धूमान् यद्यदन्यद्वितं च |८|
दीप्ते रोगे पैत्तिकं संविधानं कुर्यात् सर्वं स्वादु यच्छीतलं च ||८||

In Dipta Roga, pitta mitigating (balancing) treatments shall be done. All (external or internal) measures, foods and activities shall be done using sweet and cold things (having sweet taste and cold quality).

Treatment of Nasanaha

नासानाहे स्नेहपानं प्रधानं स्निग्धा धूमा मूर्धबस्तिश्च नित्यम् |
बलातैलं सर्वथैवोपयोज्यं वातव्याधावन्यदुक्तं च यद्यत् ||९||Snehapana (oleation) is the main treatment of Nasanaha. Apart from this, snigdha dhuma (unctuous smoking) and Murdabasti (oil pooling over the head) shall be done daily. Balataila shall be used in all ways (drinking, massage, unctuous enema, oil pooling / oil therapies over the head etc). The other oils (anutaila etc) explained in the context of Vatavyadhi should also be brought into use.

Treatment of Nasasrava

नासास्रावे घ्राणतश्चूर्णमुक्तं नाड्या देयं योऽवपीडश्च तीक्ष्णः |
तीक्ष्णं धूमं देवदार्वग्निकाभ्यां मांसं वाऽऽजं युक्तमत्रादिशन्ति ||१०||

In Nasasrava, Avapidana Nasya should be administered with strong drugs filled in nadi (tubular equipment) as Pradhamana. Apart from this, tiksna dhuma (strong smoking) with Devadaru and Citraka should be done. Food should be taken along with meat or meat juice (soup) of goat.

Treatment of Nāsāśoşa 

नासाशोषे क्षीरसर्पिः प्रधानं सिद्धं तैलं चाणुकल्पेन नस्यम् ||
सर्पि: पानं भोजनं जाङ्गलैश्च स्नेहः स्वेदः स्नैहिकश्चापि धूमः ।।११।।

Nasya in Nasasosa – Below mentioned medicaments / recipes are the most important ones to be used as nasal drops in Nasasosa –

        Ksira Sarpi – ghee extracted from butter obtained by churning of milk

        Anu Taila

Other beneficial measures –

        Sarpi pana – drinking of medicated ghee

        Jangala mamsa – foods, mainly meat of animals which live in desert like regions

        Sneha – oleation

        Swedana – sudation and

        Snaihika dhuma – inhalation of unctuous smoke (lubrication smoke)

शेषान् रोगान् प्राणजान् सन्नियच्छेदुक्तं तेषां यद्यथा संविधानम् ।।१२।।

Treatment of other diseases of the nose should be done in accordance with the stipulated procedure (related to doshās etc).

 इति श्रीसुश्रुतसंहितायामुत्तरतन्त्रान्तर्गत शालाक्यतन्त्रे नासागतरोगप्रतिषेधो नाम त्रयोविंशोऽध्यायः ।।२३।।

Thus ends the twenty third chapter by name Näsaroga Pratişedha in

Uttara Sthana of Suśruta Samhita.
