Sushruta Samhita Uttaratantra Chapter 9 Vatabhisyanda Prtisedha (Treatment of Abhisyanda of Vata Origin)


The 9th chapter of Uttaratantra of Sushruta Samhita is named as Vatabhisyanda Prtisedha Adhyaya. This chapter deals with the treatment of Abhisyanda of Vata Origin.

अथातो वाताभिष्यन्दप्रतिषेधं व्याख्यास्यामः ||१||
यथोवाच भगवान् धन्वन्तरिः ||२||

We will now expound Vatabhisyanda pratisedha – treatment of Abhisyanda (conjunctivitis) produced by Vata; as revealed by the venerable Dhanvantari.

General line of treatment of Abhisyanda and Adhimantha

पुराणसर्पिषा स्निग्धौ स्यन्दाधीमन्थपीडितौ |
स्वेदयित्वा यथान्यायं सिरामोक्षेण योजयेत् ||३||
सम्पादयेद्बस्तिभिस्तु सम्यक् स्नेहविरेचितौ |
तर्पणैः पुटपाकैश्च धूमैराश्च्योतनैस्तथा ||४||
नस्यस्नेहपरीषेकैः शिरोबस्तिभिरेव च |

Persons affected by Abhisyanda  and adhimantha caused by Vata should be given oleation using old ghee, followed by sudation. Then venepuncture shall be done according to appropriate procedure (and bloodletting done). Following this, the patient should be administered oily purgatives (or oleation followed by purgation).  Enema should be given to those administered with purgation. Tarpana, putapaka, dhuma and ascyotana therapies should be done. Nasya, Pariseka and Sirobasti therapies should also be administered.

वातघ्नानूपजलजमांसाम्लक्वाथसेचनैः ||५||
स्नेहैश्चतुर्भिरुष्णैश्च तत्पीताम्बरधारणैः |
पयोभिर्वेसवारैश्च साल्वणैः पायसैस्तथा ||६||
भिषक् सम्पादयेदेतावुपनाहैश्च पूजितैः |

Pariseka (pouring liquids on the eye) should be done with decoctions prepared with vata mitigating drugs, juice (broth) of meat of animals of lands (marshy regions) and water and decoction prepared with sour drugs. Four fats (ghee, oil, fat and bone marrow) should be made lukewarm. In this sterile cloth pads (gauze) should be soaked and these pads should be placed on the eyes. Doing upanaha (fomentation) by the physician using milk, minced meat, salvana or payasa (milk pudding) is praised (useful).

ग्राम्यानूपौदकरसैः स्निग्धैः फलरसान्वितैः ||७||
सुसंस्कृतैः पयोभिश्च तयोराहार इष्यते |

Foods of the below mentioned kind should be consumed –

        soup of meat of domestic, marshy and aquatic animals,

        made unctuous (using oil and ghee),

        should be added with juices of fruits and milk and

        should be processed well by boiling them with appropriate herbs and consumed

तथा चोपरि भक्तस्य सर्पिःपानं(ने)प्रशस्यते ||८||
त्रिफलाक्वाथसंसिद्धं केवलं जीर्णमेव वा |
सिद्धं वातहरैः क्षीरं प्रथमेन गणेन वा ||९||
स्नेहास्तैलाद्विना सिद्धा वातघ्नैस्तर्पणे हिताः |

After taking the meals, the person should drink –

        ghee boiled and prepared with Triphala Kashaya (decoction of Triphala) or

        plain old ghee

For Tarpana, milk boiled and prepared with vata mitigating herbs or herbs belonging to Vidaryadi Gana group of herbs prepared without adding fats and oil is beneficial and hence shall be used.

स्नैहिकः पुटपाकश्च धूमो नस्यं च तद्विधम् ||१०||
नस्यादिषु स्थिराक्षीरमधुरैस्तैलमिष्यते |

Lubricating types of putapaka, dhuma and nasya should be brought into use. For this purpose, oil boiled with Sthira, Ksira (milk) and Madhura Gana – herbs belonging to the sweet group (herbs predominantly having a sweet taste) should be used.

एरण्डपल्लवे मूले त्वचि वाऽऽजं पयः शृतम् ||११||
कण्टकार्याश्च मूलेषु सुखोष्णं सेचने हितम् |

For Pariseka – showering / irrigating and Ascyotana – eye drops – goat’s milk boiled and processed with tender leaves, roots and bark of Eranda – Ricinis communis should be used.

For Secana i.e. sprinkling or showering – milk boiled and prepared with paste of the roots of Kantakari should be used.

सैन्धवोदीच्ययष्ट्याह्वपिप्पलीभिः शृतं पयः ||१२||
हितमर्धोदकं सेके तथाऽऽश्च्योतनमेव च |

Below mentioned recipe is also useful for Pariseka and Ascyotana –

Milk boiled with the below mentioned ingredients added with half its quantity of water –

        Saindhava,

        Udicya,

        Yastyahva and

        Pippali

हीबेरवक्रमञ्जिष्ठोदुम्बरत्वक्षु साधितम् ||१३||
साम्भश्छागं पयो वाऽपि शूलाश्च्योतनमुत्तमम् |

Below mentioned recipe is the best one for Ascyotana to relieve pain –

Goat’s milk should be boiled and processed with –

        Hribera,

        Vakra,

        Manjistha and

        Udumbara bark

This milk is added with water and used for Ascyotana.

मधुकं रजनीं पथ्यां देवदारुं च पेषयेत् ||१४||
आजेन पयसा श्रेष्ठमभिष्यन्दे तदञ्जनम् |

Below mentioned recipe is used as Anjana – collyrium in the treatment of Abhisyanda –

Ingredients

        Madhuka,

        Rajani,

        Pathya and

        Devadaru

These herbs are macerated with goat’s milk and used as Anjana.

गैरिकं सैन्धवं कृष्णां नागरं च यथोत्तरम् ||१५||
द्विगुणं पिष्टमद्भिस्तु गुटिकाञ्जनमिष्यते |

The below mentioned ingredients are successively macerated in water –

        Gairika,

        Saindhava,

        Krsna and

        Nagara – double in quantity

After that, they are made into gutika – pills.

This pill should be used as Anjana – collyrium.

स्नेहाञ्जनं हितं चात्र वक्ष्यते तद्यथाविधि ||१६||

Using Snehanjana following the procedure is beneficial in this disease.

Anyato vata – Vata paryaya cikitsa

रोगो यश्चान्यतोवातो यश्च मारुतपर्ययः |
अनेनैव विधानेन भिषक् तावपि साधयेत् ||१७||


and Vataparyaya also should be treated by these methods only by the physician.

पूर्वभक्तं हितं सर्पिः क्षीरं वाऽप्यथ भोजने |
वृक्षादन्यां कपित्थे च पञ्चमूले महत्यपि ||१८||
सक्षीरं कर्कटरसे सिद्धं चात्र घृतं पिबेत् |
सिद्धं वा हितमत्राहुः पत्तूरार्तगलग्निकैः ||१९||
सक्षीरं मेषशृङ्ग्या वा सर्पिर्वीरतरेण वा |२०|

One should either drink ghee before consuming food or drink milk along with food.

Also, paste and decoction of Vrksadani, Kapittha and herbs belonging to the Mahat / Brhat Pancamula group of herbs.

Ghee should be prepared with

        this paste and decoction

        milk and

        meat soup of Karkata – crab

This recipe should be consumed by the patient.

Alternatively ghee boiled and prepared with Puttura, Artagala and Agnika (Ajamoda) and milk is useful. Similarly, ghee processed with milk and Mesasringi is also useful.

Also, ghee prepared (processed) with drugs of

Suskasipaka Cikitsa 

सैन्धवं दारु शुण्ठी च मातुलुङ्गरसो घृतम् ||२०||
स्तन्योदकाभ्यां कर्तव्यं शुष्कपाके तदञ्जनम् |
पूजितं सर्पिषश्चात्र पानमक्ष्णोश्च तर्पणम् ||२१||
घृतेन जीवनीयेन नस्यं तैलेन चाणुना |
परिषेके हितं चात्र पयः शीतं ससैन्धवम् ||२२||
रजनीदारुसिद्धं वा सैन्धवेन समायुतम् |
सर्पिर्युतं स्तन्यघृष्टमञ्जनं वा महौषधम् ||२३||
वसा वाऽऽनूपजलजा सैन्धवेन समायुता |
नागरोन्मिश्रिता किञ्चिच्छुष्कपाके तदञ्जनम् ||२४||

Saindhava, Devadaru, Sunthi, juice of Matulunga boiled with ghee and added with breast milk and water should be used as Anjana (collyrium). Ghee used for drinking and doing Tarpana to the eyes are ideal. Nasya should be done with either Jivaniya Ghrta or Anu Taila.

Below mentioned treatments are beneficial for those suffering from Suskaksipaka –

Pariseka – Cold water added with rock salt is used for Pariseka.

Anjana – Below mentioned recipes are useful for collyrium –

        Anjana prepared from Rajani, Devadaru and Saindhava Lavana added with ghee and breast milk,

        Anjana prepared with Mahaushadha and breast milk or

        Anjana prepared from Vasa – muscle fat of animals……and water added with rock salt and little quantity of Nagara

पवनप्रभवा रोगा ये केचिद्दृष्टि नाशनाः।
बीजोनानेन मतिमान् तेषु कर्म प्रयोजयेत्।। 

The intelligent physician should treat all those diseases produced by vata, which destroy vision, keeping in mind this mentioned sequence (essence) of treatments.

इति सुश्रुतसंहितायामुत्तरतन्त्रान्तर्गते शालाक्यतन्त्रे वाताभिष्यन्दप्रतिषेधो नाम नवमोऽध्ययः ||९||

Thus ends the Ninth chapter by name Vatabhisyanda Pratisedha in Uttara Sthana of Susruta Samhita.
